Medical Records
Your medical records are held in the strictest of confidence. Information will not be provided to a third party (attorney, insurance company, except workman's compensation carrier) unless we have written authorization signed by you to do so.
If you need copies of your records, please contact our office as far in advance as possible to fill out a Medical Records Release form. There may be costs to copy your records and/or x-rays. We require a minimum of 48 hours notice to prepare your records.
