Workman's Compensation Injuries
Our practice accepts Alaska State Workman's Compensation insurance and Federal Work Compensation. You must have filed a claim with your employer or have an open Federal Work Compensation claim in order for us to bill them. Out of state work compensation can be billed upon approval. We will bill Workman's Comp for your visits after you fill out a Workman's Compensation Injuries Form. This form asks for the date of injury, a concise explanation of the incident, a claim number, employer's name and address, as well as insurer's name, address, phone number, and name of adjuster.
